Top Reasons We Need to Call

Items are often missing from the lab order that we MUST HAVE to issue a report:

  • In order to comply with our regulatory obligations, 2 unique IDs are required on each tube of blood/urine  and its corresponding requisition. These two IDs are typically name and D.O.B.
  • In order to apply appropriate cut-points, patient gender is needed.
  • Blood collection date is required for accurate and quality test results.
  • Please check tests to be performed.
  • Please provide patient specific ICD-9 codes and insurance information.
