Follow these steps to get started with Health Diagnostic Laboratory, Inc.:

1. Call 1-877-443-5227 to order specimen collection kits.
2. Draw blood samples and ship to Health Diagnostic Laboratory, Inc. With an appropriate diagnosis, most insurance companies provide coverage for HDL Inc.’s tests. Medicare patients will likely have no out-of-pocket cost.

• Phlebotomists need a refrigerator and centrifuge for preparing and storing blood samples and a freezer for ice packs.

1. Test results are sent to the practice either by fax, mail or electronically. HDL, Inc. can provide more than one copy of the test report if required.

• Our Personal Health Coaches have strong clinical backgrounds in cardiovascular disease management & lipid metabolism and are available to help with HDL Inc.'s test interpretation and its utilization in guiding therapy. Just ask for assistance at any time.

1. Clinicians can use test results to develop a baseline for risk characterization and monitor the effects of the possible changes in therapy.
2. Based on the clinician's treatment plan and HDL Inc.'s test results, web portals are available to patients to help patients develop personalized goals for lowering disease risk.
