Our service program is designed to help patients better understand and manage their disease through prevention, education, lifestyle changes, coordination of healthcare services and adherence to prescribed plans of care (POCs) through internet-based tools. The program is designed to monitor clinical health outcome measures and track changes in expenditures and patient compliance.

HDL, Inc. is founded on the idea that all people and their diseases are not created equally.
